It comes into play with a set amount of "loyalty counters", the number in the box at the bottom where the p/t (power/toughness) usually is on creatures. Each + or - (or, in Mr. Mind Sculptor's case, "0") is next to an ability. You can use one ability per turn and either add (as Jace's +2 ability allows) that number of loyalty counters or subtract (as his third and fourth abilities instruct) that number. If loyalty counters reach or go below 0, then the planeswalker dies. Planeswalkers can be declared targets of attacks, just as someone can say "I attack you", they can say "I attack Jace/Elspeth/Nicol Bolas/etc.". You can block with your creatures for the planeswalkers just as you can for yourself for the sake of saving him. Also, with Zendikar's "Vampire Hexmage" and other counter-removing things, you can remove counters of other players' planeswalkers. There's some more finite details, but I realized that I've explained a lot already.
